Client Intake Form

By booking services with GLOW ON THE GO BY M, you acknowledge and agree to the following terms and conditions:

Services Provided: I, Madeleine Mahoney, am a licensed and certified mobile aesthetician offering the following services:

• Eyelash Extensions

• Lash Lifts & Tints

• Eyebrow Waxing & Tinting

• Brow Lamination

• Facials

Health and Safety Acknowledgment:

1. Health History: I have informed the aesthetician of any relevant health conditions, including allergies, sensitivities, or any medications I am currently taking that may affect the services provided. I acknowledge that it is my responsibility to inform the aesthetician of any changes to my health.

2. Patch Test: I understand that certain services (e.g., lash extensions, brow tinting) may require a patch test to check for possible allergic reactions. I will undergo a patch test prior to the full treatment if necessary and take full responsibility for any allergic reactions if I decline the patch test.

3. Contraindications: I confirm that I am not currently pregnant, breastfeeding, or undergoing treatments that may interfere with the services provided, unless stated otherwise by my physician.

Liability Release:

1. Eyelash Extensions & Lifts: I understand that the process of eyelash extensions and lash lifts/tints involves the use of adhesives and chemicals that may cause irritation, discomfort, or allergic reaction. I agree to release GLOW ON THE GO BY M and the aesthetician from any liability or claims resulting from my decision to receive these services.

2. Eyebrow Services: I acknowledge that eyebrow waxing, tinting, and lamination may result in temporary redness, irritation, or swelling. I understand the risks involved, including possible allergic reactions or skin sensitivities, and I release the aesthetician and GLOW ON THE GO BY M from liability.

3. Facials: I understand that facials involve the use of various skincare products and techniques. I accept responsibility for notifying the aesthetician of any skin sensitivities or allergies prior to the treatment. I release GLOW ON THE GO BY M from liability for any adverse reactions, including breakouts, redness, or irritation.

Mobile Service Acknowledgment:

1. Location & Setup: I confirm that I have provided accurate information regarding the location for my appointment. I understand that the mobile aesthetician will require a clean, well-lit, and safe area for services to be performed. If the area is not suitable, the aesthetician has the right to reschedule or refuse service.

2. Late Policy: I understand that I must be on time for my appointment. If I am more than 15 minutes late, the service may be shortened or rescheduled, and a late fee may apply.

Cancellation Policy: I acknowledge that I must provide at least 24 hours' notice if I need to cancel or reschedule my appointment. Failure to do so may result in a cancellation fee of 50% of service. No-shows will be charged the full service fee.

Photo Consent: I give GLOW ON THE GO BY M permission to take before and after photos of the services provided for promotional and marketing purposes. These photos may be used on social media, website, and other promotional materials.

By signing below, I acknowledge that I have read and understood this waiver, and I release GLOW ON THE GO BY M and the aesthetician from any liability related to the services provided.
